Intake Assessment & periodic Physical Assessments

It is crucial to conduct a thorough physical assessment of residents before intake to ensure that the facility is equipped to support the needs of the residents. Our intake assessments are facilitated by the Registered Nurse on Staff, the resident’s doctor or hospital input/notes as well as our community’s Certified Assisted Living Manager. The assessment results determine whether the resident will need Level 1, 2, or 3 of care. Subsequent periodic assessments are done to determine if the resident’s conditions or needs have changed.

Medication Management

Medications are an essential part of the care and well-being of seniors with acute or chronic conditions. When used appropriately, medications are effective treatments. Pain relievers, for example, help a person with arthritis to button a shirt. While medication is beneficial, it could also be hazardous if taken incorrectly, omitted, or not monitored for potential adverse drug reactions and interactions with other medications. 85% of the residents require assistance with the management of their medication. The medicines dramatically improve a resident’s quality of life.

Level 3 Licensed Community

OVERALL LEVEL OF CARE NEEDED

Level 1
Low level of care. This resident is mostly independent, but may need reminders to perform ADLs. Some may require a low level of supervision or assistance to ensure that tasks are performed correctly and safely.
Level 2

Intermediate or moderate level of care. The resident may be independent with some ADLs, such as feeding oneself, but needs help with others, such as getting dressed.

Level 3
High level of care. This individual has impairments that affect multiple ADLs and requires a comprehensive level of assistance from multiple caregivers.

24x7 Supervision by skilled and dedicated staff

All of our caregivers are hired on a W-2 basis and are thoroughly vetted, extensively trained, and closely supervised. We have our own Manager and Nurse on staff who oversee all residents’ condition, their medication, and charts consistently. The low caregiver-to-resident ratio allows us to provide undivided attention to your daily needs.
